Welcome! If you’re coming in to see us for the first time, we’d like you to know exactly what to expect. We want you to feel relaxed and comfortable with the knowledge that you’re in good hands.
Our office has been described as “modern, unique, fun and open.” The other word commonly used is “comforting.” We hope that you’ll agree. You’ll find that our staff is friendly and helpful and, along with Dr. Jim and Dr. Nancy, are completely dedicated to your health and well-being.
NEW PATIENT FORMS
Your First Visit
Before your first visit, we ask that you complete our new patient paperwork. You’ll see that there are specific online forms to fill out, including those for adults, children, those injured in auto accidents and workers compensation. Choose the appropriate form, and then you can either download and print, fill out on your computer and print, or save to your hard drive and then send as an email attachment to info@livewellmn.com.
Tour and Evaluation
Once all paperwork is collected, we’ll take you on a short tour of the office, and also offer you some refreshment in the form of organic tea or coffee and a selection of fresh vegetables and fruit. You’ll then be escorted to a private room, where Dr. Jim or Dr. Nancy will begin your evaluation, which consists of
- Consultation and examination
- Digital X-rays (if needed)
- 3-D bio structural exam
Keep in mind that our evaluations are customized for children with a lot of flexibility.
Your Second Visit
During this appointment, Dr. Jim or Dr. Nancy will provide you with a comprehensive report of findings, which is a detailed explanation of your exam findings and how they pertain to your current symptoms. He’ll let you know if your problem can be helped through chiropractic and will answer any questions that you may have.
Treatment options will also be discussed at this time as well as a proposed plan of action to correct your problem and get you out of pain in the shortest amount of time.
